I understand that in accordance with its Privacy Policy, the CCSPA will keep my Personal Information only as long as it is required to process my order and will endeavour to maintain appropriate security so as to prevent any loss, misuse, unauthorized access, disclosure, or modification of Personal Information.

Payment Terms
I understand that the CCSPA accepts the following methods of payment:
Credit Card (Visa, Master Card, American Express)
Cheque

I understand that my credit card will be charged in Canadian dollars when I place my order. If the CCSPA is not able to fulfill my order for any reason, my credit card will be refunded.

Cancellation
I understand that if I register but find I cannot attend, CCSPA must receive my written cancellation by September 8, 2010. A $50.00 cancellation fee will apply. Cancellations received after September 8, 2010 will not be refunded. However, registrations are transferable to employees of the same company.
